This purchase agreement is entered into on _____ / ____ / _____, between

1) Love Me Tender Mi-Kis agrees to sell to the above mentioned buyer, the following described dog/puppy:

2) The price of the dog/puppy is $ _______(STD. US Currency) payable prior to the delivery (except when other arrangement have been agreed upon between Buyer and Seller.)

3) A non-refundable security deposit of $500.00 is required to reserve the described dog/puppy. This security deposit is only refundable with the following exceptions: Death of dog/puppy, Genetic or Congenital defects that are determined prior to shipment/pickup of dog/puppy.

4) Buyer purchases this dog/puppy with the following conditions:

A. Buyer is purchasing this dog/puppy as a pet only _____(initial). The following conditions apply: The Buyer agrees to spay/neuter this dog/puppy by six months of age. Buyer agrees that under no circumstances will this pet be used for breeding, nor will it be given or sold for the purpose of breeding. After pet has been spayed/neutered, Buyer agrees to send Love Me Tender Mi-Kis, a certificate from veterinarian verifying spay/neutering. Upon receipt of certificate Seller agrees to provide registration certificate unless agreed upon by Seller.

B. The Buyer is purchasing this dog/puppy as a Breeder____(initial) If by 12 months old the dog does not meet standard for breeding, the conditions in paragraph C. will apply, or Buyer may choose to keep the dog/puppy as a pet and comply with restrictions for pets (see paragraph A).

C. Buyer agrees not to sell or give puppy to another owner without written permission from Love Me Tender Mi-Kis. Buyer agrees that under no circumstances will the Buyer, or his agent, sell, trade, or give this dog/puppy to a broker or commercial retailer for sale (pet shops, etc.).

D. The Buyer is aware that the Seller cannot guarantee size, ear set, or coat color (____initial). Seller does not guarantee against accidental damage of dog/puppy after delivery/pickup.

E. Love Me Tender Mi-Kis guarantees that this dog/puppy is vet certified to be in good physical condition at the time of delivery, and further guarantees that for six months after delivery the dog/puppy has no hereditary life threatening diseases.Should the puppy be diagnosed within six months of any congenital / hereditary disorder which is considered by two unassociated veterinarians, to seriously impact the quality of life, short of euthanasia, or require continuous medical treatment throughout the course of the puppy's life, Love Me Tender Mi-Kis will, upon provision of veterinary reports from both unassociated licensed veterinarians, provide the buyer with the following option:Should the buyer choose to keep the puppy, and accept partial refund, Love Me Tender Mi-Kis will, upon receipt of the above mentioned 2 vet reports, and proof of spay or neuter procedure, refund 50% of the purchase price.

F. Buyer agrees to have dog/puppy examined by veterinarian within 72 hours after delivery/pickup and forward a copy of the veterinarian’s findings to Seller to ensure the six-month health guarantee. If upon this examination (verified by veterinarian performing examination) the dog/puppy is found to have a genetic/congenital defect not detected prior to delivery, the Buyer agrees to take/send the dog/puppy back for a replacement of another dog/puppy of equal value when one is available. All Shipping Costs Of Dog/Puppy Exchanges Will Be The Responsibility Of The Buyer.
